Unsure about directions

The directions are unclear when it says place under tongue.. Does one swallow it or spit it out after keeping it under tongue for a while? From what I know of homeopathic meds keeping pellets under the tongue absorbs most of the active ingredients and you can spit out the sugar. With this do we spit the alcoholic base out? It tastes awful when swallowed. Not yet sure how effective it is either way.
